Outreach Services
Select Support Partnerships outreach service offers a wide range of support for people in their own homes in the community to access opportunities and activities to meet their individual needs.
Providing support in the home is all about delivering assistance when and where it is needed most. Our support workers can provide as much or as little support as required. We can support with anything from cooking, cleaning, changing bed linen and doing laundry, administration of medication and access to the local community.
Depending on the level of support required, we can provide 1:1 or 2:1 staffing, which can be long term or short term.
Fundamental to achieving this is our commitment to person centred support. This means giving every person a service which is uniquely tailored to their specific needs. By getting to know service users, their friends and their families, we can create a lifestyle plan which suits their individual needs and helps fulfil their goals and aspirations.
The outreach service assists and enables the people we support to achieve aspirations, build confidence, increase independence and live fulfilled lives.
We support people to:
- Access the local community.
- Attend college.
- Attend day centre activities.
- Attend appointments.
- Visit family.
- Attend courses.
- Prepare healthy meals.
- Attend voluntary work.
- Attend social events.
- Day trips out.
- Partake in activities that interest them i.e. football and concerts.
- Complete weekly shopping.
- Reduce social isolation.
